Online Health Declaration Instruction
Upon instruction from the Ministry of Health, everyone entering a hospital must complete a Health Declaration following:
*METHODE 1: via app Vietnam Health Declaration
*METHODE 2: via website
Step1: Access the link or scan QR Code to delcare according to area:
- Binh Duong branch: https://tokhaiyte.vn/
- Saigon branches: https://kbyt.khambenh.gov.vn
Step 2 (for only Binh Duong area): Enter your phone number to receive OTP and Enter OTP from “IT- antiNCOVI” message
Step 3 (for only Binh Duong area): Choose “For domestic guests”
Step 4: Fill in personal information
Step 5: Fill in screening information
Step 6: Submit the form
Step 7: Rceive and save QR Code for check-in/check-out at our hospitals
*Note: If you are not from Vietnam country, please kindly provide:
– Passport with latest date of entry
– Quarantine completion certificate
– Negative COVID-19 Test result
